Guide to submitting your proof of COVID-19 vaccination + booster

We have reopened our Dojo safely for indoor classes. We offer “Aikido with Contact” Classes for fully vaccinated Adults and Teens, in which we train with normal physical contact. However, face-masks and advance registration are required (no drop-ins).

These Contact Classes are strictly limited to Aikidoists who are fully vaccinated against COVID-19 and have uploaded proof of vaccination to their student account. We welcome local ASC members to these classes, as well as visiting Aikidoists from other dojos.

To maintain a safe Dojo environment for training together, here are the requirements for participation in the ASC Contact Classes:

  • Certify that you are fully vaccinated by uploading your vaccination proof, plus any booster shots, online (directions below).

  • Reserve your space in each ASC class in advance, space permitting.

  • Sign the dojo’s new Liability Waiver form upon first entry to the dojo building.

  • No drop-in visitors or exceptions to the vaccination requirements.

We strongly encourage everyone to get a Covid booster shot as soon as you are eligible, to protect yourself and others in the community.

How to certify your vaccination status

You must be fully vaccinated in order to sign up for “Aikido with Contact” classes. Please follow the steps below to submit your proof of vaccination.


Uploading proof of vaccination:

(use the same steps to verify COVID-19 booster)

  1. Sign In or Create Account in MindBody at https://clients.mindbodyonline.com/classic/ws?studioid=548143

  2. Once logged in, click on Documents in the menu within the MY INFO tab.

  3. Click Choose File to select the appropriate file with your valid proof of vaccination and click Choose for Upload.

    Note: pdf, png, jpg, or jpeg file types accepted.

  4. Next, click the blue Upload button to complete your upload.

    Note: once successfully uploaded, you will see the file listed below the upload button.

  5. Finally, please email info@aikidosantacruz.org to notify us that you submitted your proof of vaccination or booster. Once reviewed and approved, you will receive a confirmation email notifying you that you are now able to sign-up for classes designated only for those who are fully vaccinated.

FAQ

 

When am I considered fully vaccinated?

  • 2 weeks after your second dose in a 2-dose series, such as Pfizer or Moderna vaccine.

  • 2 weeks after a single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine.

    

What proof of vaccination is accepted?

A photograph or scan of any one of the following items will be accepted:

  • Valid CDC-issued Vaccination Card

  • Vaccination record from your Healthcare provider.

  • Signed letter from your primary care provider certifying you received a COVID-19 vaccine (must include vaccination type and date administered).

About Children/Teens:

ASC classes for Children ages 6-9 and 9-12 continue, with masks and physical distance required. No Covid vaccination for young children is possible at this time, but we encourage vaccination when it becomes available, to protect everyone’s health.

Teens who are fully vaccinated, upload their proof of vaccination, and whose parents have signed the new ASC liability release form, may reserve space in the Aikido with Contact classes - for Teens, and also the Adult classes.

What precautions are being taken to ensure safety in the Dojo?

We have implemented a wide array of health and safety measures to help protect everyone in the Dojo. These steps include requiring face masks of everyone in the building, Covid vaccination of all Adults and Teens, increased airflow and ventilation, updated mat-cleaning protocols, and other cleaning and disinfecting procedures in alignment with the most recent CDC information and CDPH guidance.

In our current programs for indoor classes we are limiting enrollment in classes, access to communal spaces, and overall building access.

We will review and revise our policies as more guidance emerges from health agencies and authorities.
